Medically complex situations

Does your loved one have significant health challenges?

  • Perhaps the person you care for has Parkinson’s disease. They may struggle with constrained movements, frequent falls, inability to speak at an audible volume, limited facial expression, constipation, or other uncomfortable manifestations of the disease.
  • Or maybe your relative is diabetic. They need constant monitoring, a vigilant diet, and ideally an active lifestyle to keep their metabolism working in optimal form. They may also be dealing with the depression that frequently accompanies this diagnosis.
  • Then again, there may be multiple conditions. COPD and dementia, cancer and heart failure. In no place is it written that lightning does not strike twice! Older adults frequently have to juggle conflicting regimens and try to maintain a sense of self that does not define them completely by their diagnoses.

If you are looking for an in-home caregiver who will be a partner in care, give us a call. Our caregivers are adept at implementing complex medical recommendations while also supporting your loved one to continue engaging in activities that have meaning to them. Many of our caregivers have advanced medical degrees in their country of origin. Because they are direct hires by you, they can provide an extended level of care at no extra expense: Perhaps a Hoyer lift is required, for example, or frequent use of a nebulizer, or tube feeding to meet nutritional needs. (No upsells to an advanced level of care, as is common with most home care agencies or registries.)

Aging Life Care® Management. In addition to home care, the owners of Home Care Match LLC are Aging Life Care Managers, meaning they bring holistic insight into the caregiving process. If you are seeking seamless support in the form of monitoring and care coordination, you have the option of hiring a care manager from Urban Eldercare to go with your loved one to medical visits. They understand the eldercare system and can ensure that specialists are coordinating with each other and that your relative’s wishes are an integral part of the treatment plan.

Concierge doctors. If you are not sure your relative is getting the attention needed from current physicians, we are glad to give you the names of concierge doctors who, like us, personalize their care. For instance, they offer longer visits and same- or next-day appointments. Some even make house calls!
